Calcium Carbonate vs. Calcium Citrate: Which One Should You Actually Take?

Calcium supplements aren't all the same just because the bottle says "calcium" on the front. The two forms you'll run into most — carbonate and citrate — are actually pretty different in how much elemental calcium you get per pill, how you're supposed to take them, and how your stomach handles them.

Calcium carbonate: more calcium per pill, but conditional

Carbonate is about 40% elemental calcium by weight, the highest of the common forms, so you need fewer or smaller pills for the same dose. It's also cheaper to make, which is why it shows up in most budget calcium products and multivitamins. The catch: it needs stomach acid to absorb properly, so you really should take it with food, and it's more likely to cause bloating or constipation if your gut is sensitive to it.

Calcium citrate: lower dose per pill, easier absorption

Citrate is only about 21% elemental calcium by weight, so you need roughly double the pill mass to hit the same dose. But it doesn't rely on stomach acid the same way carbonate does, so you can take it with or without food. It's usually the better choice for older adults or anyone on an acid-reducing medication, since those meds lower stomach acid and can seriously cut into how well carbonate gets absorbed.

Why this is genuinely worth filtering for

A label that just says "calcium" doesn't tell you which of these two you're getting, and they're not interchangeable in real-world use for the reasons above. Someone on a proton-pump inhibitor taking carbonate on an empty stomach could be absorbing a lot less calcium than the label suggests. It's exactly the kind of form-level detail that's nearly impossible to catch unless you can filter for it directly.
